Evaluation Report

Organizations selected to receive grant awards will be required to complete a Grant Evaluation Report six (6) months after receipt of a grant. The report should include a completed Grant Evaluation Report form, an itemized list of expenses met with funds received from Ocean State Charities Trust, and copies of paid invoices.
